Trademark Evaluation Checklist

Provide the name of word mark and brief description of the name.

If there is a design or combination mark, upload a clear image (JPEG/PNG).

Briefly describe the related goods or services. If applicable, please attach the list of the specific goods/services.

List the countries or regions of use or planned use.

Full legal name and contact details of the owner.

  1. First Use Date Anywhere: The earliest date you began using the trademark in any location
  2. Frist Use Date in Commerce: The First Use in Commerce refers to the date when the trademark was first used in interstate commerce (between states) or international commerce (between the U.S. and a foreign country).

For goods, submit labels, packaging, or website screenshots with a “buy now” button. For services, provide ads, brochures, or website screenshots showing the trademark with service details. Include the URL and date for website submissions. Use JPEG, PNG, or PDF formats.

If you have any current or historical registrations or cancellations, please provide the corresponding numbers.

Attach any office actions, refusals, opposition notices, or infringement details.

Copyright Evaluation Checklist

Full name, contact details, nationality, address.

  1. Do all works share the same copyright claimant?
  2. Is the author the same as the copyright owner (claimant)?

  2. Type (literature, art, music, photographs, Illustrations, Novels, Scripts, Articles, or Contributions to Periodicals, etc.)
  3. How many works?
  4. Completion date

Brief summary, work samples (text excerpts, images).

Explanation of originality, including any co-creators (if applicable)

Details on commercial use or online publication, with links/screenshots:

  1. Published or Not
  2. Date of Publication
  3. All or Partly Publication

Provide details on infringement, including the infringer’s information and supporting evidence.

Provide the registration number if the work has already been registered.

Your most recent resume/CV.

Your country of birth, and your spouse’s country of birth if applicable

Other relevant materials, such as media reports, awards, or professional memberships. (please do not send confidential information).
